Branching Out – Wellspring Healthy Living Centre
What is it?
One to One support to help get you engaged with physical activity and community based activities. Helping you to find what’s right for you, helping to break down barriers and building your confidence to do it.
Who is it for?
Adults living in BS5 – Lawrence Hill – who would like support in getting more active
What will I do?
You can self-refer or be referred by your GP. You will meet your social prescribing worker on a one to one basis, who will help you identify what you would like to do to become more active, help identify what gets in the way and help you to set goals and build motivation. You will meet with your social prescribing worker on a regular basis to review your goals and set new ones.
How much is it?
The social prescribing service is free. There may be a cost to some activities that your social prescribing worker signposts you to; for example a walking group is free, however there is a small weekly cost for going to an exercise class.
How long will it last?
We generally see clients for 3 to 4 sessions.
What will I achieve from being part of the project?
We aim to help you feel more confident, achieve your goal, be more physically active, more socially connected and feel healthier.
What happens after the project?
Your social prescribing worker will support you to set goals and signpost you to different activities in services in the community that you can continue with in the long-term, following your one to one sessions.
